Complete the form in full. This involves personal details, doctor’s report, social workers report, surety form etc.

Only completed forms will initiate with the screening process by management.
If your application form is not complete you will be contacted to collect your form for completion.
After the screening process, you will be contacted for an appointment with the Manager and medical Sister in charge.
This help us to determine on what floor the applicant can be accommodated (as we only have stairs and ramps not lifts).
Thereafter your application will be submitted to the Governing Board Selection Committee for final approval.
The Selection Committee meets monthly, or as needed and scheduled by the Manager.
The decision of the Selection Committee is communicated to the applicant.
If a room is available a service level agreement is signed with the applicant and the family before admission.
